If the product will be potentially tracked in a computer system using ISBT 128, new product description codes may be obtained by contacting the ICCBBA, Inc office or checking the ICCBBA, Inc Web site. To request a new product description code, a Product Description Code (Blood Components) Request Form found in the back of the ISBT 128 Product Code Database document or in the password-protected section of the Web site. This form must be completed.
In the US, requests should be sent to Frank Berry, staff liaison for the AABB Information Systems Committee (frank@aabb.org). For further information refer to Section 5 of the ISBT 128 Product Code Database document.
