In the first version of the US Consensus Standard, there was more than one possible location for the US license number. It is now recommended that the US license number appear in the lower left quadrant, along with the product code or in the upper left quadrant below the name of the facility.
The advantage of placing it in the lower left quadrant is that when a facility is licensed, not all products that they produce are licensed. Therefore, the US license is a function of the product code. If the original, licensed product is modified (e.g., irradiated), then it makes sense that the license number is over labeled since the daughter product may not be licensed. Further, such modification may occur at a facility that is not licensed.
The US Consensus Standard has identified the location for the FDA license number on the label. Facilities must print their FDA license number on all licensed products. The original US Consensus Standard also allowed the Collection facility to place the US License number in the lower right quadrant. It has since been determined that this location needs to be reserved for a facility that subsequently modifies the product.
