To request a new product description code that is not already in the database, a Product Description Code Request Form may be found in the back of the ISBT 128 Product Code Database document or in the password-protected section of the Web site. This form must be completed and submitted to a nationally designated organization or individual and then to ICCBBA. For further information, refer to Section 5 of the ISBT 128 Product Code Database document.
